Saw Palmetto for BPH: Does the Berry Help a Weak Stream?
Saw palmetto for BPH has not beaten placebo in the best trials. NIH says it is probably not helpful, a Cochrane review of 32 trials found no gain in peak flow or symptom score, and CAMUS found none at triple doses. It is mild on the stomach, so the question is cost and delay, not danger.

What is saw palmetto and why is it used for BPH?
Saw palmetto is the berry of a low palm, Serenoa repens, sold for years to men with a weak stream and night-time bathroom trips.
BPH is a non-cancerous growth of the prostate. It can slow the stream and wake you at night (NIDDK, Enlarged Prostate (BPH)). Popularity is not proof: the test is whether men on saw palmetto did better than men on a dummy capsule.

Does saw palmetto work for an enlarged prostate?
Not in the largest, best-run studies. Three sources agree on that, and they use different methods.
NIH calls saw palmetto probably not helpful for BPH. A 2023 review of 27 studies found little or no benefit when it was taken alone (NCCIH, Saw Palmetto).

Is saw palmetto harmless to try?
Mostly yes. Side effects are mild: stomach upset, dizziness or headache (NCCIH, Saw Palmetto).
Blood in the urine or being unable to empty the bladder needs a doctor, not a supplement. Keep taking prescribed medicine.

Saw palmetto and BPH questions
Is saw palmetto the same thing as a prostate shrinker?
No. Neither the Cochrane review nor CAMUS found that saw palmetto improved prostate size or urine flow versus placebo (Tacklind et al., Cochrane 2012) (Barry et al., JAMA 2011 (CAMUS)).
Why do some men say saw palmetto helped them?
Symptoms vary, and expecting relief colors scores. In CAMUS the placebo group improved slightly more (Barry et al., JAMA 2011 (CAMUS)).
How long did the longest saw palmetto trial run?
CAMUS ran 72 weeks in 369 men, raising the dose from 320 to 960 mg a day (Barry et al., JAMA 2011 (CAMUS)).
Can saw palmetto replace a BPH prescription?
No evidence supports that. Nobody should stop a prescription on their own; ask the prescriber (AUA BPH Guideline (Sandhu 2023 amendment)).
Is saw palmetto fruit different from extract?
Trials mostly used extract. The label prints fruit with no extract ratio, so trial doses do not map across.
