Guide: male vitality safety

Are Male Enhancement Supplements Safe? Hidden Drugs, Epimedium, Tribulus and Testosterone Claims

Quick answer

Are male enhancement supplements safe? Some are not. FDA has found products sold as all natural that held hidden sildenafil and tadalafil, which can drop blood pressure dangerously with nitrates. Herbs such as epimedium and tribulus have thin safety data. Protoflow lists both, so the honest answer is to ask a pharmacist first.

The short answer01

Are male enhancement supplements safe?

Some are, many are unproven, and a few have been found to hide prescription drugs. The label alone cannot tell you which kind you hold.

NIH says natural does not always mean safe. Side effects are most likely at high doses, in place of prescribed medicines, or when many supplements are combined (NIH ODS, Dietary Supplements: What You Need to Know). It also notes that some sexual-enhancement products may contain undisclosed prescription drugs (NIH ODS, Dietary Supplements: What You Need to Know).

Protoflow is a male-oriented capsule with a lime-and-green label and a male symbol on the side strip. Its front says "Optimal Flow Support†" and never claims sexual results. Still, it holds two libido herbs, epimedium and tribulus, and that is why this guide exists.

Hidden drugs02

Can a natural supplement contain hidden Viagra or Cialis?

Yes. FDA tests found sildenafil and tadalafil, the active drugs in Viagra and Cialis, inside products that did not list them.

In a public notice dated February 5, 2020, FDA reported that its lab found undeclared sildenafil and tadalafil in a sexual-enhancement product called Alpha-Male. FDA says products sold as all natural for sexual enhancement, weight loss or body building may contain hidden drugs (FDA, Alpha-Male public notification).

The danger is the mix. These drugs can interact with nitrates such as nitroglycerin and lower blood pressure to dangerous levels, a concern for people with diabetes, high blood pressure, high cholesterol or heart disease (FDA, Alpha-Male public notification). MedlinePlus carries the same nitrate warning for tadalafil and sildenafil (MedlinePlus, Tadalafil).

What we can and cannot say about Protoflow

Its label lists no ED drug, and we found no sign of one. But no lab report is published, and a lot number is a seller reference, not proof. Anyone on nitrates should ask a pharmacist first. Report a bad reaction to FDA through MedWatch (FDA, Alpha-Male public notification).

The two herbs03

What do we know about epimedium and tribulus safety?

Not enough. Both are sold for libido, and both have case reports of harm and little human proof of benefit.

Epimedium, or horny goat weed, is 250 mg a serving in Protoflow. MSK says use for sexual dysfunction is traditional, studies are needed to verify effects, and its safety still needs to be determined (MSK, Epimedium).

Tribulus is 120 mg. The testosterone-like claim is not clearly supported, and human data on sexual function are mixed (MSK, Tribulus terrestris).

HerbReported problemsCaution
EpimediumSweating, feeling hot, rapid irregular heartbeat, mood changes (MSK, Epimedium)Avoid with heart disease; care with hormone-sensitive cancers
TribulusStomach irritation; case reports of liver, kidney and nerve toxicity and a 72-hour painful erection (MSK, Tribulus terrestris)May add to blood pressure and diabetes drugs; men with prostate cancer should ask first

Testosterone claims04

Do tribulus and other herbs boost testosterone?

The evidence does not clearly support it. Low testosterone is diagnosed with blood tests, not by a label.

Tribulus is marketed on the belief that it acts like testosterone. MSK says the evidence does not clearly support that, human testosterone studies are mixed, and one athlete study found it did not improve androgen status (MSK, Tribulus terrestris).

True low testosterone, called hypogonadism, shows up as low sex drive, muscle loss or infertility. MedlinePlus says it is diagnosed with an exam and blood tests, and it is harder to judge in older and obese men (MedlinePlus, Male Hypogonadism). Treatment is typically testosterone replacement from a provider (MedlinePlus, Male Hypogonadism). A capsule is not a stand-in for a test.

The Protoflow sales page also lists a bonus ebook with testosterone in the title. We have not read it, and we make no claim about what it says.

ED causes05

What causes erectile dysfunction in the first place?

ED has physical, medical, emotional and lifestyle causes, and it can be an early sign of another health problem.

NIDDK lists diseases such as diabetes and kidney disease, medicines including some antidepressants, blood pressure drugs and pain relievers, emotional factors, and lifestyle causes such as inactivity, heavy drinking and smoking. Hardened arteries, high blood pressure and stroke are named among the vascular causes. NIDDK says 30 to 50 million U.S. men have ED and that it is not a routine part of aging (NIDDK, Erectile Dysfunction).

That is a reason to see a doctor before reaching for an herb. A supplement can hide a heart warning, and fixing the cause comes first.

Before you buy06

How do I check a male vitality supplement against my medicines?

Look up the class of medicine you take, then ask a pharmacist about each plant on the panel.

  • Warfarin, apixaban (Eliquis), aspirin and other blood thinnersAsk first

Ginkgo with warfarin is linked to more major bleeding, ginseng's effect on warfarin is mixed, and large doses of vitamin E can add to bleeding. Ask your prescriber before you start. (NCCIH, Herb-Drug Interactions) (NCCIH, Ginkgo) (MSK, Ginseng (Asian)) (NIH ODS, Vitamin E)

  • Nitrates, blood pressure and heart medicines (including digoxin)Ask first

Lab data suggest hawthorn may interfere with digoxin and add to antiplatelet effects; epimedium has been linked to rapid irregular heartbeat and should be avoided in heart disease; cayenne may alter blood pressure drugs. Ask your doctor first. (MSK, Hawthorn) (MSK, Epimedium) (MSK, Cayenne)

  • Erectile dysfunction medicines (sildenafil, tadalafil, vardenafil)Ask first

Tadalafil is not for people who take nitrates, and both drug families have their own warnings. Epimedium is sold for the same goal. Tell your doctor about every product, and note that FDA has found hidden sildenafil and tadalafil in some 'natural' sexual-enhancement products. (MedlinePlus, Tadalafil) (MSK, Epimedium) (FDA, Alpha-Male public notification)

  • Tamsulosin, alfuzosin, finasteride, dutasterideUse care

No interaction with the label ingredients is documented in the sources we checked. Finasteride and dutasteride lower PSA, and saw palmetto is not shown to change it. Do not swap a prescribed prostate medicine for a supplement. (MedlinePlus, Tamsulosin) (NCI, PSA Test Fact Sheet) (NCCIH, Saw Palmetto)

  • Diabetes medicines (insulin, glipizide, metformin)Ask first

Ginseng may lower blood sugar too far with insulin or sulfonylureas, and damiana may affect blood sugar in animal studies. Ask your doctor first and know the signs of a low. (MSK, Ginseng (Asian)) (MSK, Damiana)

  • Gout medicines (allopurinol, colchicine)Use care

Inosine turns into uric acid, which may be hazardous for people at risk of gout. Mention it to the prescriber who manages your gout. (UofM Health (Healthwise), Inosine)

  • Prostate cancer treatment or active surveillanceAsk first

A large trial linked 400 IU a day of vitamin E to a higher prostate cancer risk. The label's 13 mg is much lower, but ask your urologist or oncologist about any supplement. (NIH ODS, Vitamin E) (NCI, PSA Test Fact Sheet)

  • Medicines that can cause urinary retention (antihistamines, decongestants, opioids)Use care

These can worsen trouble emptying the bladder, which is the problem some men hope a prostate supplement will fix. A supplement does not offset them; ask your pharmacist. (NIDDK, Urinary Retention)

A short list built from NIH, MSK, FDA and trial sources, not a full interaction check. Bring the bottle or the Supplement Facts panel to your pharmacist. Keep taking prescribed medicine.

  • Does the product promise sexual results in words or pictures?
  • Is the maker or lab named, with a test report?
  • Do you take nitrates or blood pressure medicine?
Keep taking prescribed medicine

Do not stop a heart, blood pressure or prostate medicine for a supplement. Blood in the urine or an erection lasting more than 4 hours needs urgent care (MedlinePlus, Tadalafil). See the medicines guide.

FAQ07

Male vitality supplement safety: your questions answered

How do I know if a supplement contains hidden sildenafil?

You cannot tell from the label. FDA found undeclared sildenafil and tadalafil by laboratory analysis (FDA, Alpha-Male public notification). Warning signs are promises of fast sexual results and no named lab. Ask a pharmacist and report problems to FDA.

Why are nitrates dangerous with ED drugs?

Sildenafil and tadalafil can interact with nitrates such as nitroglycerin and lower blood pressure to dangerous levels (FDA, Alpha-Male public notification). MedlinePlus says your doctor will probably tell you not to take tadalafil with nitrates (MedlinePlus, Tadalafil).

Is epimedium safe for men with heart disease?

MSK advises avoiding epimedium with heart disease, citing a case of rapid irregular heartbeat in a man with heart failure (MSK, Epimedium). Protoflow has 250 mg per serving, so ask your cardiologist first.

Can tribulus cause liver or kidney problems?

There are case reports of liver, kidney and nerve toxicity, and one 72-hour painful erection (MSK, Tribulus terrestris). These are rare reports, not proof of frequency. Protoflow has 120 mg of tribulus fruit per serving.

How is low testosterone confirmed?

With an exam and blood tests for testosterone and related hormones (MedlinePlus, Male Hypogonadism). Symptoms such as low sex drive and muscle loss can have other causes, so a provider reviews them before any treatment.

When is an erection an emergency?

MedlinePlus says an erection lasting longer than 4 hours, or sudden vision or hearing loss, needs urgent medical care (MedlinePlus, Tadalafil). Go to the ER rather than waiting it out.

Can ED be a sign of heart problems?

It can. NIDDK lists hardened arteries, high blood pressure and stroke among causes and says ED may be a symptom of another health problem (NIDDK, Erectile Dysfunction). See a doctor before treating it yourself.
