Saw Palmetto in Protoflow: 84.5 mg a Day, and What the Trials Found
Saw palmetto in Protoflow is 84.5 mg of fruit per 2-capsule serving. The large trials used 320 mg a day or more and found little or no benefit for BPH symptoms. Cochrane reviewed 32 trials, and the CAMUS trial tested up to 960 mg. NCCIH says saw palmetto is probably not helpful for BPH.

The ingredient01
What is saw palmetto in Protoflow?
Saw palmetto is the one herb on the Protoflow label with a long research record for the prostate. The panel lists it as Saw Palmetto (Fruit), 84.5 mg per 2 capsules.
Saw palmetto is the berry of Serenoa repens, the best-known prostate herb.
It is one of 14 lines. Only cayenne at 20 mg, oat straw at 32 mg and inosine at 50 mg are smaller. The sales page names saw palmetto but prints no amount (Protoflow info page). The Supplement Facts panel does (Protoflow bottle label).

Why it is the key line
Men who search for "protoflow saw palmetto" want the herb with the most prostate trials behind it. The trials were disappointing, and the label dose sits well below them.
Dose check02
How much saw palmetto is in Protoflow?
Each serving of 2 capsules holds 84.5 mg of saw palmetto fruit, so each capsule holds 42.25 mg.
The label allows 2 capsules a day, so 84.5 mg is the whole day's amount. The trials gave 320 mg a day or more (Barry et al., JAMA 2011 (CAMUS)). And 84.5 divided by 320 is about 0.26, so the label holds roughly a quarter of the lowest trial dose. The top CAMUS dose of 960 mg is about 11 times the label amount.
The label does not say whether the line is ground fruit or an extract. The trials used extract, so the real gap may be wider.
Extra capsules would break the label's "do not exceed suggested use" line and push niacin further over its 35 mg limit (NIH ODS, Niacin).
The evidence03
Does saw palmetto work for BPH?
The best evidence says little or no benefit. NCCIH calls saw palmetto probably not helpful for BPH.
NCCIH points to a 2023 review of 27 studies that found little or no benefit for BPH symptoms when saw palmetto is given alone. Two NIH-funded trials, one at a standard dose and one at up to three times that dose, did not improve symptoms either (NCCIH, Saw Palmetto).
| Source | What it covered | What it found |
|---|---|---|
| NCCIH (NCCIH, Saw Palmetto) | 2023 review of 27 studies | Little or no benefit for BPH symptoms when given alone |
| Cochrane (Tacklind et al., Cochrane 2012) | 32 trials, 5,666 men, 4 to 72 weeks | No better than placebo for peak flow (0.40 mL/s) or symptom score (0.25 points) |
| CAMUS (Barry et al., JAMA 2011 (CAMUS)) | 369 men aged 45 or older, 72 weeks, 320 to 960 mg a day | Symptom score fell 2.20 points on saw palmetto and 2.99 on placebo |
| AUA guideline (AUA BPH Guideline (Sandhu 2023 amendment)) | 2023 amended guideline | Many studies had weak methods; two double-blind trials showed no benefit over placebo |
In CAMUS, the placebo group improved 0.79 points more than the saw palmetto group.
Higher doses04
Would a higher dose of saw palmetto help?
Not in the CAMUS trial. It raised the dose to three times the starting level and still found no gain over placebo.
CAMUS stepped the dose from 320 mg to 640 mg to 960 mg a day at weeks 24 and 48. Saw palmetto was no better than placebo on any secondary outcome (Barry et al., JAMA 2011 (CAMUS)). Cochrane found the same at double and triple doses: no better urinary flow or prostate size in men with BPH-type symptoms (Tacklind et al., Cochrane 2012). The dose gap on the Protoflow label is real, but even trial-sized doses gave little or no benefit.

Is saw palmetto safe, and does it change a PSA test?
NCCIH describes saw palmetto as well tolerated, with mild and infrequent side effects. It does not appear to affect PSA readings.
The side effects named are digestive symptoms, dizziness and headache. It may be unsafe in pregnancy or while breastfeeding (NCCIH, Saw Palmetto).
PSA is a blood test used in prostate checks. BPH and prostatitis can raise it, and finasteride lowers it (NCI, PSA Test Fact Sheet). Tell your doctor about every supplement before a PSA test.

Next steps06
What helps urinary symptoms more than a supplement?
Lifestyle steps come first in the AUA guideline, and a checkup finds the cause.
The AUA calls lifestyle measures reasonable first-line care: less fluid before bedtime, less caffeine and alcohol, and timed voiding (AUA BPH Guideline (Sandhu 2023 amendment)). Doctors can add medicines, such as alpha blockers, for symptoms that bother you (NIDDK, BPH Treatment).
- Cut evening fluids, caffeine and alcohol (MedlinePlus, Nocturia).
- Keep taking any prescribed medicine. A supplement does not stand in for it.
Blood in the urine, burning with fever, groin or back pain, or trouble emptying the bladder needs a doctor soon. Being unable to urinate at all is an emergency: call 911. Read prostate support options and prostate basics.
Verdict07
Where does saw palmetto leave Protoflow?
Saw palmetto is a small, well-studied line with weak results, so it should not be the reason to buy.
The label is honest about the amount. The research is the problem: the herb was tested at higher doses than the label holds and showed little or no benefit for BPH. See prostate supplements compared.
Fair points
- The 84.5 mg amount is printed
- Mild side effects per NCCIH
Weak spots
- About a quarter of the lowest trial dose
- Trials found little or no BPH benefit
- No trial of the Protoflow mix

FAQ08
Saw palmetto in Protoflow: common questions
Does saw palmetto shrink an enlarged prostate?
No trial we read shows that. Cochrane found no better prostate size or urinary flow at double and triple doses in men with BPH-type symptoms (Tacklind et al., Cochrane 2012), and NCCIH calls it probably not helpful for BPH (NCCIH, Saw Palmetto). An enlarged prostate needs a checkup and a plan from a doctor.
How many milligrams of saw palmetto are in Protoflow?
A 2-capsule serving holds 84.5 mg of saw palmetto fruit, which is 42.25 mg a capsule. The label allows one serving a day, so 84.5 mg is the daily amount.
Is 84.5 mg of saw palmetto enough to matter?
Probably not. The lowest dose in the trials was 320 mg a day, nearly four times the label amount (Barry et al., JAMA 2011 (CAMUS)). Even at 960 mg a day, men did no better than on placebo, so more milligrams would not settle the question.
What did the CAMUS trial find about saw palmetto?
It found no benefit over placebo. CAMUS followed 369 men for 72 weeks at 320 to 960 mg a day. Symptom scores fell 2.20 points on saw palmetto and 2.99 on placebo, and no secondary outcome favored the herb (Barry et al., JAMA 2011 (CAMUS)).
Does saw palmetto change PSA test results?
NCCIH says it does not appear to affect PSA readings (NCCIH, Saw Palmetto). Still, BPH, infection and some medicines move PSA (NCI, PSA Test Fact Sheet), so tell your doctor about every supplement, including Protoflow, before a test.
What are the side effects of saw palmetto?
They are usually mild and infrequent: digestive upset, dizziness and headache (NCCIH, Saw Palmetto). It may be unsafe in pregnancy or while breastfeeding.
Why is saw palmetto in a supplement if the trials were negative?
Because it is the best-known prostate herb and is sold for prostate and urinary comfort. The bottle never names the prostate, and the research has not kept pace with the marketing (NCCIH, Saw Palmetto).
