Enlarged Prostate Treatment Options: Lifestyle, Medicines and the Saw Palmetto Evidence

Quick answer

Enlarged prostate treatment options start with lifestyle changes and watchful waiting for mild symptoms, then medicines such as alpha blockers (tamsulosin) or 5-alpha reductase inhibitors (finasteride), then procedures or surgery. Saw palmetto, the best-known herb, did no better than placebo in large trials. Your doctor chooses the plan.

The ladder01

What are the treatment options for an enlarged prostate?

The options run from watching and changing habits, to medicines, to procedures and surgery. Doctors move up the ladder only as far as your symptoms require.

NIDDK describes it this way. For mild BPH, doctors may use watchful waiting with yearly checkups and lifestyle changes. Medicines come next. Procedures and surgery are options if medicines do not help or symptoms are severe (NIDDK, BPH Treatment).

The American Urological Association guideline agrees that behavioral and lifestyle steps are reasonable first-line care for all patients (AUA BPH Guideline (Sandhu 2023 amendment)). A supplement is not on the ladder as a treatment. It sits beside it, as something to mention.

The usual order of care for BPH symptoms, in general terms. Your doctor sets yours.
StepWhat it isWho it suits
Watchful waitingYearly checkups, no drugs (NIDDK, BPH Treatment)Mild symptoms that do not bother you
Lifestyle stepsLess evening fluid, caffeine and alcohol; timed voiding (AUA BPH Guideline (Sandhu 2023 amendment))Nearly everyone, alongside other care
Alpha blockersRelax muscle in the prostate and bladder neck (NIDDK, BPH Treatment)Bothersome moderate to severe symptoms (AUA BPH Guideline (Sandhu 2023 amendment))
5-alpha reductase inhibitorsSlow growth or lower gland size (NIDDK, BPH Treatment)Men with an enlarged gland (AUA BPH Guideline (Sandhu 2023 amendment))
Procedures or surgeryWater vapor therapy, a prostatic urethral lift or surgery (NIDDK, BPH Treatment)Medicines fail or symptoms are severe

Lifestyle02

Which lifestyle changes help urinary symptoms?

The AUA lists limiting fluids before bedtime or travel, cutting back on caffeine, alcohol and bladder irritants, avoiding constipation, staying active and losing weight if needed.

It also lists timed voiding, which means going on a schedule rather than waiting for the urge (AUA BPH Guideline (Sandhu 2023 amendment)). None of that costs anything, and each step can be tried this week.

NIDDK adds a caution. You may stop drinking a few hours before bed to reduce night trips, but only if your clinician suggests it, and never to the point of dehydration (NIDDK, Treatments for Bladder Control Problems). It also suggests a bladder diary and slowly lengthening the time between bathroom visits (NIDDK, Treatments for Bladder Control Problems).

  1. 01

    Get the cause checked

Night-time trips and a weak stream have several causes. A visit sorts out BPH from infection, diabetes or a medicine side effect.

  2. 02

    Change the evening

Cut back on fluids, caffeine and alcohol late in the day, and only if your clinician agrees. Keep a short bladder diary.

  3. 03

    Ask what medicines fit

If symptoms still bother you, an alpha blocker, a 5-alpha reductase inhibitor or both may be offered.

  4. 04

    Set a review date

Return at a fixed time to judge whether anything changed, whether a pill or a habit.

A diary also gives you something concrete to show the doctor. Three nights of trips, plus what you drank after dinner, is worth more than a guess.

Watchful waiting03

What is watchful waiting for an enlarged prostate?

Watchful waiting means no drug yet. You keep a yearly checkup, change a few habits and see whether symptoms change.

Doctors may suggest it for mild BPH (NIDDK, BPH Treatment). The point is that growth alone is not a problem. Symptoms that bother you are.

It is not the same as ignoring things. The signs that end the waiting are blood in the urine, burning with fever, pain or an inability to empty the bladder. Those need care soon, and not being able to urinate at all needs a 911 call. The prostate basics guide lists them.

Medicines04

What do tamsulosin and finasteride do?

Tamsulosin is an alpha blocker that relaxes muscle in the prostate and bladder. Finasteride is a 5-alpha reductase inhibitor used for men with an enlarged gland.

Tamsulosin is prescribed for BPH by relaxing muscles in the prostate and bladder. It may cause dizziness, lightheadedness and fainting, especially when you stand up fast or are starting it. Tell your doctor you take it before any eye surgery (MedlinePlus, Tamsulosin).

Finasteride may take at least 6 months to improve BPH symptoms. It can cause erection trouble, lower desire and ejaculation problems, and these may continue after stopping. MedlinePlus also notes it may increase the risk of high-grade prostate cancer (MedlinePlus, Tamsulosin). It lowers PSA, so your doctor needs to know you take it.

NIDDK says an alpha blocker plus a 5-alpha reductase inhibitor may work better than either alone (NIDDK, BPH Treatment). The AUA reserves the second kind for men with an enlarged gland (AUA BPH Guideline (Sandhu 2023 amendment)). Which one fits depends on gland size, PSA and your other health. That is a prescriber's decision.

Keep taking prescribed medicine

Do not stop or change tamsulosin, finasteride or any other prescription because of a supplement. If you want to add one, tell your doctor and pharmacist first. See Protoflow and medicines.

Saw palmetto05

Does saw palmetto work for an enlarged prostate?

Probably not. NCCIH says saw palmetto is probably not helpful for BPH, and large trials found no better results than placebo.

A 2023 review of 27 studies found little or no benefit for BPH symptoms when saw palmetto is given alone (NCCIH, Saw Palmetto). Two NIH-funded trials, one at the standard dose and one at up to three times that dose, did not improve symptoms either (NCCIH, Saw Palmetto).

The three main pieces of evidence on saw palmetto and BPH.
SourceSizeResult
NCCIH review of 27 studies (NCCIH, Saw Palmetto)27 studies, 2023Little or no benefit alone
Cochrane review (Tacklind et al., Cochrane 2012)32 trials, 5,666 menNo better than placebo for peak flow or symptom score
CAMUS trial (Barry et al., JAMA 2011 (CAMUS))369 men, 320 to 960 mg a daySymptom score fell 2.20 points on saw palmetto and 2.99 on placebo

The AUA adds that many saw palmetto studies have methodological weaknesses, and that two double-blind placebo-controlled trials found no benefit over placebo (AUA BPH Guideline (Sandhu 2023 amendment)). NCCIH calls it well tolerated, with mild stomach upset, dizziness or headache, and says it does not appear to affect PSA (NCCIH, Saw Palmetto).

So the herb is unlikely to harm and unlikely to help. That is a fair reason to skip it, and a fair reason to be wary of a capsule that leans on it.

Protoflow06

Where does Protoflow fit among these options?

It does not sit on the ladder. Protoflow is a dietary supplement, and its saw palmetto line is 84.5 mg a serving, against 320 mg a day at the low end of the trials.

A two-capsule serving holds 84.5 mg of saw palmetto fruit. The CAMUS men started at 320 mg of extract and rose to 960 mg (Barry et al., JAMA 2011 (CAMUS)). The label dose is about a quarter of the lowest trial dose, and even the high doses did not beat placebo.

FAQ07

Treatment options: your questions answered

What is the first treatment for a mildly enlarged prostate?

Watchful waiting with yearly checkups and lifestyle changes, such as less fluid, caffeine and alcohol before bed (NIDDK, BPH Treatment). The AUA calls lifestyle measures reasonable first-line care for all patients (AUA BPH Guideline (Sandhu 2023 amendment)). Medicines come later if symptoms bother you.

How long does finasteride take to shrink an enlarged prostate?

MedlinePlus says finasteride may take at least 6 months to improve BPH symptoms (MedlinePlus, Tamsulosin). It is meant to slow growth or lower gland size, and it has side effects such as erection trouble, so your doctor monitors it.

What is tamsulosin used for, in simple terms?

Tamsulosin relaxes muscles in the prostate and bladder to ease urine flow in BPH. It can cause dizziness or fainting, especially when standing up fast or when starting (MedlinePlus, Tamsulosin). Tell your eye surgeon you take it before any eye surgery.

Can an alpha blocker and finasteride be taken together?

Sometimes. NIDDK says an alpha blocker plus a 5-alpha reductase inhibitor may work better than either alone (NIDDK, BPH Treatment). Only a prescriber can judge that, based on gland size, PSA and your other health.

What does watchful waiting involve for BPH?

No drug, a yearly checkup and a few habit changes (NIDDK, BPH Treatment). You watch your stream and night-time trips. It ends if you see blood in the urine, get fever with burning, or cannot empty the bladder.

Is saw palmetto approved to treat BPH?

No. FDA does not approve supplements, and NCCIH says saw palmetto is probably not helpful for BPH (NCCIH, Saw Palmetto). The CAMUS trial of 369 men found no benefit over placebo at doses up to 960 mg a day (Barry et al., JAMA 2011 (CAMUS)).

How much saw palmetto did the BPH trials use?

The CAMUS trial used extract at 320 mg a day, rising to 640 mg and then 960 mg (Barry et al., JAMA 2011 (CAMUS)). Protoflow's label has 84.5 mg of saw palmetto fruit per 2 capsules, so it is well below those doses.

Should I cut caffeine and alcohol for night-time urination?

It is a reasonable first step. The AUA lists cutting caffeine, alcohol and bladder irritants, and limiting fluids before bedtime (AUA BPH Guideline (Sandhu 2023 amendment)). NIDDK says to ask your clinician before cutting fluids, and never to the point of dehydration (NIDDK, Treatments for Bladder Control Problems).
