What Does the Prostate Do? BPH, Symptoms, PSA and Red Flags

Quick answer

The prostate is a walnut-sized gland below the bladder that adds fluid to semen. With age it often grows, which is called BPH, and can squeeze the urine channel, causing a weak stream and night-time trips. BPH is not cancer. Some urinary signs, such as blood in the urine, need a doctor, not a supplement.

The gland01

What does the prostate do?

The prostate is a small gland that sits just below the bladder and wraps around the urine channel. Its job is to make part of the fluid in semen.

The urine channel, called the urethra, runs through the middle of the gland. When the gland grows, it presses on a tube that was already narrow. A young man's prostate is about the size of a walnut.

The label desk reads supplement panels. We are not doctors, so this guide sticks to what the NIH and other public sources say (NIDDK, Enlarged Prostate (BPH)). The Protoflow bottle never names the prostate, though its sales page does.

BPH02

What is BPH, or an enlarged prostate?

BPH stands for benign prostatic hyperplasia.

BPH is very common. NIDDK says it affects about 5% to 6% of men aged 40 to 64 and about 29% to 33% of men 65 and older (NIDDK, Enlarged Prostate (BPH)).

Size and symptoms do not always match. One man with a big prostate may have mild symptoms, while another with a modest one feels every trip (NIDDK, Enlarged Prostate (BPH)). That is why a doctor asks about your stream, not only about the gland.

BPH is not the only cause. Prostatitis, inflammation of the gland, causes pain, burning or fever in some forms (NIDDK, Prostatitis). Diabetes, infection and some medicines can look the same.

Symptoms03

What are the symptoms of an enlarged prostate?

The usual signs are a weak or stop-and-start stream, trouble getting started, feeling the bladder is never empty, urgency, and waking at night to urinate.

NIDDK lists these: trouble starting urination or emptying the bladder, a weak or interrupted stream, frequent urination, urgency, nocturia and pain with urination (NIDDK, Enlarged Prostate (BPH)).

Picture a hose with a foot on it. The flow dribbles, and some water stays behind. A bladder that never fully empties fills sooner, so the trips come more often.

Night-time trips04

Why do I urinate so often at night?

Waking to urinate is called nocturia, and an enlarged prostate is only one cause. Evening fluids, caffeine, alcohol, sleep apnea and some medicines are others.

MedlinePlus lists many causes: a lot of fluid in the evening, caffeine and alcohol with or after dinner, urinary infection, enlarged prostate, kidney disease, diabetes, heart failure, diuretics, leg swelling and sleep disorders (MedlinePlus, Nocturia). A doctor works through that list, so you should not guess.

A week of notes on when you drink, have coffee and wake can point to the cause.

A 24-hour day: 2 capsules with water in the morning, a walk at midday, fewer fluids and no caffeine in the evening, and 7 to 9 hours of sleep2461218
  1. Morning, about 8:00

2 capsules with a glass of water: the label's suggested use. The label gives no time of day, so pick one you can keep.

  2. A walk at midday

Staying active is on the AUA's list of first-line habits for urinary symptoms; it is not a Protoflow effect.

  3. Evening, from about 6:30

Urology sources suggest cutting caffeine and alcohol and not drinking a lot in the last hours before bed, if your clinician agrees. Never limit fluids to the point of dehydration.

  4. Night, 7 to 9 hours

Count your night-time bathroom trips for a week. Waking to urinate has many causes, from an enlarged prostate to diabetes, heart failure and sleep apnea, so bring the number to your doctor.

Habits from the AUA BPH guideline (AUA BPH Guideline (Sandhu 2023 amendment)), NIDDK (NIDDK, Treatments for Bladder Control Problems) and MedlinePlus (MedlinePlus, Nocturia). The capsule time is a suggestion; follow your label.

If waking continues over several days, the trips bother you, or urination burns or has blood in it, MedlinePlus says to contact a provider (MedlinePlus, Nocturia). The next guide covers what doctors offer once you have been checked.

The PSA test05

What is a PSA test and what do the numbers mean?

PSA is a protein made by the prostate. A blood test measures it, and a high result is a reason for more tests, not a diagnosis.

The National Cancer Institute says PSA is made by normal prostate cells as well as cancer cells. BPH and prostatitis can push it up. So can an infection, a recent biopsy, vigorous exercise and ejaculation, the last two briefly (NCI, PSA Test Fact Sheet).

A PSA above 4.0 ng/mL is generally called abnormal, but no single number separates safe from unsafe (NCI, PSA Test Fact Sheet). About 6% to 7% of men get a false alarm each time they are screened. Only about 25% of men biopsied for a high PSA turn out to have cancer (NCI, PSA Test Fact Sheet).

Finasteride and dutasteride lower PSA, so a man on either medicine should tell his doctor before a test (NCI, PSA Test Fact Sheet).

What the screening panel recommends. This is general public guidance, not advice for you.
AgeUSPSTF gradeWhat it says
55 to 69CAn individual choice after talking through benefits and harms with a clinician (USPSTF, Prostate Cancer Screening)
70 and olderDRecommends against PSA-based screening (USPSTF, Prostate Cancer Screening)

Its named harms are false positives, overdiagnosis and treatment side effects (USPSTF, Prostate Cancer Screening).

Red flags06

Which urinary signs mean see a doctor now?

Blood in the urine, burning with fever and chills, pain in the groin or back, or not being able to empty the bladder need a doctor. Not being able to urinate at all needs emergency care.

NIDDK says to seek care right away if you cannot urinate at all, have painful urgent urination with fever and chills, see blood in the urine, or have great pain in the lower belly or urinary tract (NIDDK, Enlarged Prostate (BPH)). Sudden retention can be life threatening (NIDDK, Urinary Retention).

Chronic retention is quieter: incomplete emptying, small frequent voids, a slow stream or leaking (NIDDK, Urinary Retention). Antihistamines, decongestants, opioids and NSAIDs can cause or worsen it (NIDDK, Urinary Retention).

  1. Blood in the urinePink, red or cola-colored urine
  2. Burning, fever or chillsPossible infection of the bladder or prostate
  3. Cannot empty the bladderA weak stream that is getting worse fast, or leaking
  4. Pain in the groin, testicles or lower backSudden or severe pain needs a doctor today
  5. Cannot urinate at allWith a swollen, painful lower belly: emergency

Most casesWaking at night, a slower stream or urgency are common as men age, and BPH is the usual cause. Still book a visit: the same symptoms can come from infection, diabetes, medicines or cancer, and a PSA test or exam is a talk to have with a clinician.

Call 911Or go to the emergency room if you cannot urinate at all, or have severe pain in the lower belly or groin. Sudden inability to pass urine is a medical emergency.

Signs from NIDDK (NIDDK, Urinary Retention) and (NIDDK, Enlarged Prostate (BPH)) and MedlinePlus (MedlinePlus, Nocturia). A supplement is never a reason to wait.

Keep taking prescribed medicine

Do not stop or change a prescription because of a supplement or an article. Ask the doctor who prescribed it.

What next07

What should I do about an enlarged prostate?

Book a visit, get the causes sorted, then choose from lifestyle steps, medicines and procedures. A supplement is an optional extra, not step one.

For mild BPH, NIDDK says doctors may use watchful waiting with yearly checkups and lifestyle changes such as limiting fluids, caffeine and alcohol before bed (NIDDK, BPH Treatment). Medicines and procedures come next if symptoms bother you.

FAQ08

Prostate basics: your questions answered

What does the prostate gland do for a man?

It makes part of the fluid in semen and sits around the urine channel just below the bladder. It does not make urine. Growth with age can narrow the flow (NIDDK, Enlarged Prostate (BPH)).

Is an enlarged prostate the same as prostate cancer?

No. BPH is non-cancerous growth of the gland (NIDDK, Enlarged Prostate (BPH)). A doctor tests rather than guesses.

Can you have an enlarged prostate and no symptoms?

Yes. Prostate size and symptoms do not always match, so a large gland can cause little trouble while a small one causes plenty (NIDDK, Enlarged Prostate (BPH)).

What raises a PSA level besides cancer?

BPH, prostatitis, infection, a recent biopsy, vigorous exercise and ejaculation can all raise PSA (NCI, PSA Test Fact Sheet).

Is a PSA of 4.0 always a problem?

No. Above 4.0 ng/mL is generally called abnormal, but no single cutoff separates normal from abnormal (NCI, PSA Test Fact Sheet). About 25% of men biopsied after a high result are found to have cancer, so many high readings are not cancer.

Should every man over 55 get a PSA test?

The USPSTF calls it an individual decision for men 55 to 69, made with a clinician after weighing benefits and harms. It recommends against PSA screening from age 70 (USPSTF, Prostate Cancer Screening).

Can an enlarged prostate stop you from urinating?

Yes, in some men. Sudden inability to urinate with lower belly pain is acute retention and can be life threatening, so it needs emergency care (NIDDK, Urinary Retention). Call 911 if you cannot pass urine at all.

Is waking up to urinate at night always the prostate?

No. Evening fluids, caffeine, alcohol, sleep apnea, diabetes, heart failure and diuretics can all cause it (MedlinePlus, Nocturia). See a provider if it continues for days, bothers you, burns or shows blood.
