Hawthorn is the berry of the Crataegus shrub, a traditional heart herb. Cayenne is the hot pepper fruit, and its irritant compound is capsaicin. Both are printed with exact amounts on the panel (Protoflow bottle label).

Does hawthorn help blood pressure or the prostate?
The data on hawthorn and high blood pressure are conflicting, and no source we read links hawthorn to the prostate.
MSK reports conflicting data on hawthorn and high blood pressure. It adds that an overdose may cause low blood pressure and an irregular heartbeat (MSK, Hawthorn). NCCIH's hawthorn page, from April 2025, says there is little or no quality evidence for most conditions, and that no study tested use beyond 16 weeks.
That second point matters for a product sold by the month. A 90- or 180-day pack runs well past the longest period anyone has studied.
Hawthorn has a long record as a heart herb. A heart herb is not a prostate herb, and Protoflow has not been tested in a trial. The bottle says only "Optimal Flow Support†", with the FDA dagger (FDA, Structure/Function Claims).
For cayenne, MSK describes its effects on thermogenesis and appetite as small (MSK, Cayenne). At 20 mg a day, the line is closer to seasoning than to a studied dose.

Can you take hawthorn with digoxin or blood pressure pills?
Ask your prescriber first. Lab data suggest hawthorn may interfere with digoxin and may add to blood pressure drugs, antiplatelets and anticoagulants.
MSK explains that hawthorn contains digoxin-like compounds. Lab data suggest it may raise side effects of antiplatelet and anticoagulant drugs, and may change how the body handles drugs cleared by the CYP3A4 enzyme. How much this matters in people is unknown, and the page says people on heart medicines should use hawthorn only under a physician's direction (MSK, Hawthorn).
It also says not to use hawthorn in place of heart failure therapy. That line is worth reading twice. Protoflow is a supplement, not a heart medicine, and nothing on the bottle suggests otherwise.

Does cayenne irritate the stomach?
It can. MSK names stomach irritation and referred gastric pain as the common oral effects of cayenne.
A case report in the MSK source describes stomach cell loss and bleeding in healthy volunteers who ate capsicum. That is a case report and not a trial, but it is why a person with ulcers, reflux or a sensitive stomach should ask first (MSK, Cayenne).
Blood pressure medicine
MSK says cayenne may alter the effects of antihypertensives, and may increase the cough that some ACE inhibitors cause (MSK, Cayenne).
Transplant medicine
The same source says to avoid cayenne with cyclosporin. Animal data hint at antiplatelet activity, though the human relevance is unknown (MSK, Cayenne).
Taking capsules with water and a meal is a sensible habit with any stomach-irritating herb, though the label only says to take two capsules daily with water. Cayenne is also listed as contraindicated in pregnancy and breastfeeding. The label is for adults only.
