Saw Palmetto Research: What a 32-Trial Cochrane Review Concluded
Saw palmetto research is large and mostly disappointing. A Cochrane review pooled 32 trials and 5,666 men and found no gain over placebo in peak urine flow or symptom score. NIH cites a 2023 review of 27 studies with little or no benefit. A pooled review is stronger than any one trial.

What did the saw palmetto reviews find?
On flow and on symptom score, saw palmetto did not separate from placebo.

Both ranges cross zero, which means the data fit "no effect". Doubling and tripling the dose did not improve flow or prostate size either (Tacklind et al., Cochrane 2012).

Is the saw palmetto research flawless?
No. The AUA guideline says many saw palmetto studies have methodological weaknesses (AUA BPH Guideline (Sandhu 2023 amendment)).

What is a Cochrane review?
A summary that pools randomized trials on one question using a set method, so one odd study cannot steer the answer (Tacklind et al., Cochrane 2012).
Why does a range that crosses zero mean no effect?
If the true difference could be zero or even negative, the data cannot show saw palmetto beat placebo (Tacklind et al., Cochrane 2012).
Did researchers test saw palmetto with other herbs?
NIH's review looked at saw palmetto given alone (NCCIH, Saw Palmetto). Trials of mixes are rarer and cannot isolate one herb.
Could a newer study change the answer?
